Returning to nursing or midwifery- restoring to the register or roll

If you have been previously registered, enrolled or authorised to practise midwifery only in Queensland and wish to restore your name to the register or roll you need to:

IMPORTANT: It is essential that you carefully read the Checklist for application for restoration prior to completing your application form to ensure that you complete all of the required paperwork and supply the correct supporting documentation.

1. Download the Application form, Checklist for application for restoration and Schedule of fees and charges or contact us for paper copies.
2. Complete Application form. Please ensure you signature at Item 10 - Statutory Declaration, is witnessed by a justice of the peace, commissioner for declarations, solicitor or notary public. Other signatories are not acceptable.
3. Post by mail, or deliver in person to our office, the following:
   • the completed Application form with original signatures
   • the correct application and processing fees as outlined in the Schedule of fees and charges checklist.

When your application has been assessed you will receive written advice of the outcome. If you have not practised nursing within the five years preceding your application you will be required to demonstrate nursing competence through the Competence Assessment Service.  If you have previously been a registered nurse you may apply for enrolment if you wish.

Please note: A random audit process is conducted on all applications.  Where a declaration of competence is made and your application is selected for audit you will be required to provide documentary evidence to support your declaration. 

QNC will also seek a criminal history report from the Queensland Police Service on all restoration applicants.  If you have spent 12 months or more outside Australia in the last 10 years, you will be required to provide an overseas police clearance/s.  For more information, see the information sheet on criminal history.

What happens if I am selected for audit? 

If you are selected for audit, you will need to complete an Audit response form - RN, EN, Midwife-only and return it to Council. 

If you are also applying to restore your midwifery endorsement, you will need to demostrate midwifery competence through completion of an Audit response form - midwifery endorsement.
